Writing in npj Breast Cancer, researchers present IICM+, which blends clinicopathologic data, transcriptomics, and multiscale pathology embeddings to predict distant recurrence in HR-positive, HER2-negative, node-negative early breast cancer. Trained on TAILORx cases (n=2808) and tested on a holdout set (n=1621), it reached a C-index of 0.735 for overall distant recurrence and a high-versus-low hazard ratio of 5.25, remaining prognostic after adjustment for recurrence score. Why it matters: slides may refine chemo choices where gene assays leave discordant risk. Caveat: holdout results are not a prospective trial, and Caris employees coauthored the study.
